Assessment Educator Committee Application Form
In an effort to clean up and update our Assessment Educator Committee database and to ensure we have the most current information for potential committee participants, ADE's Assessment Section has launched a newly revised Assessment Educator Committee Application. The new application link has been published on our website and is included below.

All Arizona educators who would like to be considered as a participant for future Assessment Educator Committees must complete the new application to be included in our database. Our old database is going away, and your information will not transfer to our new database automatically.

Even if you have recently served on an educator committee or if you have recently submitted your application, you must submit a new application using the new form linked below if you would like to be considered for future committees.
