Special Paper Version Test Requests
Open Now through February 24, 2023

Special Paper Version (SPV) test requests for AASA (Grades 3-8), AzSCI (Grades 5, 8, and 11/Cohort 2024), and ACT Aspire (Grade 9/Cohort 2026) should be submitted to ADE for review and approval using this secure online form: https://www.azed.gov/assessment/spv2023. Data submitted in this form is secure and accessible only to ADE Assessment staff.

Arizona offers the following SPV test formats to meet the needs of students who cannot access the test content through computer-based testing: braille, large print, and regular print paper tests. Please see the SPV Test Request Process Quick Guide for additional information and for the steps necessary to request an SPV test for a student.

District Test Coordinators may submit requests or can authorize School Test Coordinators or other staff members to submit requests. An automated email will be sent to the District Test Coordinator and the person filling out the form to let submitters know that the information was received and will be reviewed. ADE's Assessment team will then follow up by email after the review is complete or to request additional documentation, if needed.

SPV test requests will be accepted through February 24, 2023; however, we encourage you to submit requests early to allow time for submitting additional documentation, if required. Contact us if a student has an injury or illness after February 24, 2023 that necessitates a paper test.

Please note: SPV test requests and other accommodations for ACT (Grade 11/Cohort 2024) are to be submitted directly to ACT using ACT's processes and are not to be submitted directly to ADE for review and approval.
Additional Accommodations Requests
Open Now through February 24, 2023

Additional Accommodations requests may now be submitted to ADE for review and approval for AASA, AzSCI, and ACT Aspire using this secure online form: https://www.azed.gov/assessment/achievement_aa_fy23

Some examples of Additional Accommodations that should be submitted to ADE for review include the American Sign Language (ASL) form of the test, assistive technology needed by a student during testing, or testing accommodations required due to a student injury as described in the Achievement Assessments Accommodations Manual.

Additional Accommodations requests SHOULD NOT include Universal Test Administration Conditions, Universal Tools, or other accommodations identified as allowable in the Achievement Assessments Accommodation Manual, located on ADE's Assessment Accessibility page.

A request is valid for the current school year. If instructional and assessment accommodations in the student's IEP or 504 Plan are updated during the current school year after the initial submission, you will need to submit the updated IEP or 504 Plan.

Please see the new Additional Accommodations Request Process document for additional information.
